Care Solutions LLC collects nonpublic personal information (“Client Information”) to aid us in finding appropriate long-term care options for our clients. Including: information provided to us by clients, family, friends, or medical professionals.
Type of Information Collected
Care Solutions LLC may collect the following information: client preferences, current living situation, financial information, and health information.
Disclosure of Personal Information
Care Solutions LLC discloses Client Information to long-term care providers when locating long-term care options for a client. Care Solutions LLC does not disclose Client Information to prior to obtaining a signed consumer disclosure. Exceptions include those required by law and emergencies where a verbal consumer disclosure is provided. in the case of a verbal disclosure, a written consumer disclosure will be required in a reasonable time. Care Solutions LLC may provide information to third parties that offer support services as needed or requested by a client.
Limits to Disclosure
Care Solutions LLC never sells Client Information to unrelated third parties. Care Solutions LLC restricts access to Client Information only to companies that may provide services to client. Care Solutions LLC maintains physical, electronic, and procedural safeguards to protect Client Information.
Limits to Liability
Care Solutions LLC will make every effort to protect Client Information. Care Solutions LLC does not warrant or guaranty any third party’s compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Care Solutions LLC is not responsible for acts of negligence, recklessness, or unlawful acts by the third parties, their employees, or assigns.
Right to Prohibit Disclosure
Care Solutions LLC respects each Client’s privacy and rights. Clients have the right to view information collected about them by Care Solutions.
